Application Checklist

  To complete the application process, please review the checklist items listed below and submit all required supplemental forms. 

• Submit the completed application and application fee. 

Submit the following documentation:

• Standardized Test Scores

• School Attendance Report

• NWEA Maps Data ELA and Math Grades 3-8

• Disciplinary Report

• Current/Previous Report Card

• Attendance Records

*Incoming Kindergarten and Homeschool students must upload documentation to complete their application. 

*Once your application and all required documentation are received, you will be contacted to schedule an interview. 

If your student is accepted at Carlsbad Prep Academy, you will receive the information needed to complete the enrollment process.
